Sheepshead Nursing and Rehabilitation Center shall not exclude, deny benefits to, or otherwise discriminate against persons who are clients or desiring to be admitted to the facility, in accordance with the provisions of Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, Regulations of the U.S. Department of Health and Human Services issued pursuant to these statutes at Title 45 Code of Federal Regulations Parts 80, 84, 91 and 92, and New York State Executive Law Article 15 Human Rights Law Section 296. Sheepshead Nursing and Rehabilitation Center does not discriminate in any manner on the basis of race, creed, color, national origin, age, religion, sex, sexual orientation, sexual preference, military status, pregnancy, gender identity or expression, disability, blindness, marital status, citizenship, genetic information, sponsorship in admission, retention and care of residents, or any other category prohibited by applicable federal, state, or local laws and regulations. Appropriate auxiliary aids and services as well as language assistance services are available free of charge. If you believe that Sheepshead Nursing and Rehabilitation Center has failed to provide these services or discriminated in another way you can file a grievance.
